COVID-19 Guidelines and Attendee Acknowledgement

HDA and the HDA Research Foundation recognize the safety of all of those attending the Research Foundation CEO Roundtable Fundraiser at the Conrad Washington DC, as a top priority.  HDA and the HDA Research Foundation are committed to following CDC, local government, and the Conrad Washington DC COVID-19 health and safety guidelines for in-person events.

HDA COVID-19 Policy

All attendees at the 2021 HDA Research Foundation CEO Roundtable Fundraiser will be required to be fully vaccinated. Proof of vaccination will be required along with a temperature check before entering any HDA Foundation- or HDA-associated meeting room. 

Attendees must show proof of vaccination. A copy of your vaccination card must be submitted via this form by Friday, November 12, in order to attend the event. You will receive confirmation of receipt once submitted. If attendees do not submit proof of vaccination, they will not be able to enter any of the events or meetings. Additionally, temperature checks will be required before entering any HDA associated meeting on site at Conrad Washington DC.

Based on the D.C. government and hotel COVID-19 guidelines, all attendees are currently required to wear a mask in all public areas of the hotel (except when actively consuming food or beverage).

Attendee Acceptance of Terms and Conditions

By attending the CEO Roundtable in Washington, D.C., I agree to comply with all COVID-19 health and safety guidelines adopted by HDA and the HDA Research Foundation, recommended by the CDC, local government agencies, and the Conrad Washington DC, I acknowledge receipt of and have read these policies.
