COVID-19 Guidelines and Attendee Acknowledgement

HDA and the HDA Research Foundation recognize the safety of all of those attending the HDA Research Foundation CEO Roundtable Fundraiser at The Mayflower Hotel in Washington DC, as a top priority.  HDA and the HDA Research Foundation are committed to following CDC, local government, and The Mayflower Washington DC COVID-19 health and safety guidelines for in-person events.

HDA COVID-19 Policy

All attendees at the 2022 HDA Research Foundation CEO Roundtable Fundraiser will be required to be fully vaccinated (received two doses of the Moderna or Pfizer vaccine or one dose of the Johnson and Johnson vaccine by November 1, 2022). Proof of vaccination will be required before entering any HDA Foundation- or HDA-associated meeting room. 

Attendees must show proof of vaccination. If we have not previously received proof of your vaccination, you will receive an email prompting you to submit a CEO Roundtable Proof of Vaccination form by Friday, November 11, 2022 in order to attend the event. You will receive confirmation of receipt once submitted. If attendees do not submit proof of vaccination, they will not be able to enter any of the events or meetings

By attending the CEO Roundtable in Washington, D.C., you agree to comply with all COVID-19 health and safety guidelines adopted by HDA and the HDA Research Foundation, recommended by the CDC, local government agencies, and The Mayflower Hotel.
